at this point i feel like the kind doctor only wanted to feel important bcos like theres nothing stopping him from divorce and quitting but he said his reasons were "what if the commoner come looking for me" and blablabla(by what the book is describing that he's the best kind doctor, then that means that there are other kind doctors as well??) he took care of other peoples children but didnt take care pf his own and even abused those who are weaker than him and that he know is not "important" and saving those who he sees can gv him that feeling of "importance'' since it seems that he doesnt have any backbone in his house and his wife walk all over him so he crave for that "impirtant" feeling

kind doctor, for as long as u keep quiet in front of your wife, she will continue to step on your head b'coz she sees that u r weak. there r many poor people but d worst r unreasonable people like some of your patients' parents n your wife. to strife u gotta b firm so that those unreasonable people won't take advantage of u n waste your time.
